We Are
Aging as Ourselves is an innovative community-wide collaborative that provides a safe and comfortable environment for lesbian, gay, bisexual and transgender (LGBT) seniors in San Diego to tap into culturally responsive legal, social and health related services. For the first time, six mainstream and LGBT health and social service providers are working together to ensure that senior services are delivered in an LGBT culturally sensitive manner and that LGBT providers expand the types of services they offer to address specific health issues facing this hidden population.
Aging as Ourselves provides a safety net of community organizations to assist LGBT seniors with supportive services including: in-home care management including assessment of individual needs and development of specific care plans; referral to community resources for housing, home health care, nutrition services, transportation and home-delivered meals; legal assistance focusing on the specific needs of LGBT seniors; mental health counseling; HIV and health education; social activities; volunteer grocery shopping; and in-home friendly visiting for homebound seniors. In addition, Aging as Ourselves provides training and assistance to health and social service providers and recruits and trains volunteers to help them understand the unique needs of aging LGBT adults.
Aging as Ourselves is funded by both local and national funders and has been recognized by the Robert Wood Johnson Foundation as a promising model for health care and social service delivery.
